Duties and Responsibilities of a Home Health Aide/Personal Care Assistant:
  • Assist patient with personal care and activities of daily living, safety supervision, chart in the medical record, and report to the agency RN Supervisor.
  • Personal hygiene, toileting, and incontinence care
  • Bathing, shaving, hair care, and routine skin care
  • Oral hygiene including denture care
  • Dressing and undressing
  • Assistance with mobility: ambulation, transfers, positioning, and prescribed range of motion exercises
  • Prevention of pressure ulcers
  • Apply non-sterile, dry dressings to intact skin surfaces
  • Empty or change Foley catheter bag
  • Empty or change Colostomy bag
  • Care of hearing aides, glasses, and prosthetic devices
  • Nail care (filing ok, but no cutting nails)
  • Obtain and record vital signs including blood pressure, temperature, pulse and respirations
  • Meal planning, preparation, serving, and feeding
  • Measure and record “in-take” and “out-put”
  • Remind or assist with pre-poured medications
  • Light housekeeping, maintain a clean and healthy environment
  • Home safety, emergency preparedness, CPR and Heimlich maneuver
  • Chart the medical record
  • May drive/escort on errands, shopping, doctor appointments
